1. Your student roster

The roster is one row per student who started a program in the reported years, with the dates, job and exam records 5 CCR §74112(m) says a school must keep. Your file is read in this browser. Only the totals per program and year are sent to us.

What goes in each column
Student ID (required column)
Your school's own ID for the student. Never a Social Security number. 5 CCR §74112(m)(8)
Name
The student's full name. 5 CCR §74112(m)(2)
Address
The student's mailing address. 5 CCR §74112(m)(2)
Phone
The student's phone number. 5 CCR §74112(m)(2)
Email
The student's email address. 5 CCR §74112(m)(2)
Program (required column)
The program name, spelled exactly as in the questionnaire. Each program gets its own fact sheet. 5 CCR §74112(a), (m)(2)
Location (required column)
The campus or branch name, spelled exactly as in the questionnaire. Each location gets its own fact sheet. Education Code §94928(c); 5 CCR §74112(a)
Start date (required column)
The date the student started the program. 5 CCR §74112(m)(2)
Scheduled completion date (required column)
The date the student was scheduled to finish, at 100% of the published program length. This decides which year's figures the student counts in. 5 CCR §74112(d)(1), (m)(2)
Actual completion date (required column)
The date the student completed the program and was awarded the diploma or degree. Leave blank if they did not complete. 5 CCR §74112(d)(2), (m)(2); Education Code §94842
Cancelled during cancellation period
Yes if the student cancelled within the cancellation period. These students are left out of every count. 5 CCR §74112(d)(1)
Unavailable for graduation
Only for students who did not complete: died, incarcerated, or military (called to active military duty). Leave blank otherwise. Education Code §94928(g), (h); 5 CCR §74112(l)
Unavailable for employment
Only for graduates who, after graduating, died, were incarcerated, were called to active military duty, are international students who left the U.S. or cannot work here, or are continuing their education at an accredited or Bureau-approved school. Education Code §94928(f); 5 CCR §74112(l)
Unavailability dates
When the student became unavailable (and when it ended, if it did). 5 CCR §74112(m)(8)
Unavailability documented
Yes if you hold a document that shows the unavailability (for example military orders or an enrollment letter from the other school). 5 CCR §74112(m)(8)
Employed
Yes if the graduate reported a job, self-employment or freelance work after graduating. 5 CCR §74112(d)(3), (l)
Employment basis
How the job counts: soc position (a job in one of the program's listed SOC codes), same employer (same employer as before enrolling, after a promotion, a new job code, or because the program was required), or self-employed (including freelance). 5 CCR §74112(d)(3)(A)-(C)
Employer name
The employer's name, or the graduate's business name if self-employed. 5 CCR §74112(m)(3), (m)(4)
Position
The graduate's job title. 5 CCR §74112(m)(3)
SOC code
The six-digit Standard Occupational Classification code of the job, like 39-5011. 5 CCR §74112(d)(3)(A)(i), (i)(3)
Employment start date
The date the job began. 5 CCR §74112(m)(3)
Employment end date
The date the job ended, if it has ended. 5 CCR §74112(m)(3)
Annual salary
Yearly salary or wages as reported by the graduate or employer, like $32,500. 5 CCR §74112(k), (m)(3)
Hours per week
Hours worked per week, across all positions if the graduate holds more than one. 5 CCR §74112(d)(3)(A)(ii), (m)(3)
Concurrent positions
Yes if the hours come from more than one position held at the same time. 5 CCR §74112(d)(3)(A)(ii), (i)
Part-time statement signed
Yes if the graduate signed a statement that they chose part-time work. Needed to count a job of 20 to 29 hours a week. 5 CCR §74112(d)(3)(A)(ii)
Employed by school or affiliate
Yes if the employer is your school, a business your school owns, or a business that shares ownership with your school. 5 CCR §74112(i)
Employment verified date
The date you verified the job with the graduate or employer. 5 CCR §74112(l), (m)(3)
Employer address
The employer's address. 5 CCR §74112(m)(4)
Employer phone
The employer's general phone number. 5 CCR §74112(m)(4)
Employer contact name
The person at the employer who confirmed the job. 5 CCR §74112(m)(4)
Employer contact phone or email
That person's phone number and email address. 5 CCR §74112(m)(4)
Self-employment evidence
What shows the self-employment: business license, fictitious business name statement, advertising, website, business receipts, or a signed and dated attestation from the graduate. 5 CCR §74112(d)(3)(C), (m)(5)
Contact attempts
Each attempt to reach the graduate or employer: date, method and outcome. 5 CCR §74112(m)(6)
First exam date
The date of the first licensing exam the graduate could take after completing. 5 CCR §74112(j), (m)(7)
Exam result
Result of that first available licensing exam: passed, failed, did not take, or unknown. 5 CCR §74112(j), (m)(7)
